Servo Drives burning out

I have had 2 servo drives (left and right side turrets) burn out with in 1 month of each other. Severe burning at the output of the drive. ASD-A2-0721-U model.
The servo motor is operating within amperage specification.
Input to the drive is 230Vac (rated 200- 230V)
Output of drive is 110Vac but have measured 150Vac what can cause this?

How did you measure the output voltage?
